Electronic Health Record Tutorial
Below is a six-minute video that offers a sneak preview of the electronic health record (EHR) patient portal. Parents will use this secure portal to submit their student’s Health Forms. Additional information about accessing the EHR is listed below the video.
Dear Hill Parents,
Please submit your child’s annual Health Forms to the Wellness Center via Medicat's secure, online patient portal. All of our clinical service areas, including the Wellness Center, Counseling Center, and Athletic Training, utilize this EHR.
Parents will register for the portal using a User Name provided by Hill. This is the same User Name that you use for MyBackPack. You will need to use your first and last name in the exact format that you provided to the School. The format must match exactly what is in the EHR.
Please visit https://hillschool.medicatconnect.com/ to register for an account or to access your account. To register for a new account, click the “Register” button at the top right of the screen. Enter your username, email, first name, and last name. Then click "Submit."
You will receive an email to prompt you to create a password. Create a password that is at least 8 characters in length and contains an upper case, lower case, number, and special character. Please save the portal as a “favorite” in your browser for easy future reference.
Once you login to the portal with your user name and password, you will use the Forms, Immunizations, and Upload tabs to submit the required health information.

Forms tab:
There are three documents that you will need to have your child’s healthcare provider sign.
- Immunization History Form 2023-2024
- Prescription Medication Order Form 2023-2024 If your child does not take any prescription medications, you do not need to complete this form.
- Physical Exam Form 2023-2024
Please print these forms from the portal and take them to your healthcare provider at the time of your child’s physical exam. The physical exam must be dated on or after May 1, 2023. As a reminder, we will NOT accept physical exams, prescription medication order forms, or immunization history forms that are signed by a provider who is also the student’s parent.
Some forms must be completed online. Click on each form title to access the form for online completion. As you complete the form, the listing will move lower on the list, become unbolded, and appear in black font instead of blue. In that manner, you can easily track which forms still need to be completed.
Immunizations Tab:
We ask that you enter immunization dates into the portal via the “Immunizations” tab.
Entering the dates is a one-time task. You will also need to upload the signed immunization form from the "Upload" tab.
Click on the drop-down menu for "Required Vaccines." You will see a list of "Missing Requirements." Scroll down until you see the option to enter dates for the various vaccines. Refer to your child’s immunization record to enter the dates for each dose and then click "Submit." You will also do this for "COVID-19 Vaccines." Once you have entered the dates, upload the immunization record on the "Upload" tab.
Returning students only need to enter the new vaccines received since last summer.
When you submit the date of a vaccine, it will be shown as "not verified." Once the Wellness Center staff has verified dates within the EHR, this status will change to "verified."
Upload Tab:
After the instructions, this tab first lists all forms which can be uploaded on the portal (“Documents available to be uploaded”). The four that are pertinent to all students are:
- Physical Exam Form 2023-2024
- Immunization History Form 2023-2024
- Prescription Medication Order Form 2023-2024 (without a healthcare provider’s signature if the student does not take any prescription medication)
You will use the drop-down menu to select which form you are uploading, then choose a file you have saved to your computer.
As a reminder, Health Forms are due July 15. There is a late fee of $250 for students who have not completed health forms by July 15.
The Wellness Center will notify you when your child is cleared for arrival for the fall.
